Unless arrangements have been made in advance, co-payments, co-insurance, and any outstanding balances are expected at the time of service. Patients may be financially responsible for payment of all services. Regardless of insurance coverage prior to your appointment, please check your insurance information so you will be informed about referrals, co-payments, and any deductibles required at the time of the visit. We accept Mastercard, Visa, Discover and American Express.
For each visit, please bring your insurance card and photo ID, and arrive early to complete or update the necessary information forms. Please ask any questions or review your insurance information with our staff.
AESTHETICS
All services are considered, cosmetic in nature and therefore are not covered by insurance. Payment is expected at the time of treatment. Accepted forms of payment are Cash, Check, Mastercard, Visa, Discover and American Express.
